CHECKLIST FOR PASSPORT APPLICATION - RENEWAL
New Passport Application (Reissue)- Requirements
- Original Passport (including subsequent additional booklets if any)
- New Passport application form duly filled
- 3 latest passport-size photographs
- Copy of resident permit/valid visa.
- If passport is renewed after 1 year from the date of expiry, an affidavit is to be filled and additional fee is payable.
- Authority letter in favor of Indian national if applicant is not able to visit Embassy with copy of passport of authorized person.
- Minors are issued passports for 5 years or up to attaining 18 years, whichever is earlier. Passport applications, along with passport copies of parents, should be signed by both parents. Minor should affix his/her signature or left thumb impression in the “Signature Boxes”.
- Minors between the age of 15 and 18 have the option to apply for a full validity passport (ten years) by payment of fees as applicable for issue of passports to persons above 18 years of age. However, a ten-year validity passport in such cases will be issued only on receipt of police verification report.
- Additional documents requirement based on specific conditions:
- Change of signature - If your signatures have changed than an affidavit for change of signature to be submitted.
- Change of surname – There is change of surname after marriage/remarriage/divorce or you need to add your spouse name in your new passport. Please provide a self-declaration.
- Change in appearance – If your appearance has changed considerably then please submit duly attested Form J with a current photo.
- Change of address – If there is a change in address then please attach a copy of any utility bill duly attested by Notary.
- Change of Name – If there is a complete name change, addition of surname, spelling correction of name, etc., please provide copy of notification in two leading Indian Newspapers, Gazette Notification of the concerned State Govern
